Children and Teenager's Course Registration Information

Children's courses require pre-registration. To apply for a course, parents are requested to download the three forms found on the links below and send them to the course registrar.

• Application form
• Medical emergency form
• Release for use of child photo form

Please print and send the completed forms to the course registrar mentioned below for each course. Forms can be either mailed or faxed to the course registrar. After we receive your completed application and permission forms, we will review your application and contact you.

If you are unable to print the forms, please contact the registrar with your name and address and application forms will be mailed to you.
