Children's Course Registration Information

 Parents interested in sending their children to course are required to completed the below forms:

• Application form
• Medical emergency form
• Release for use of child photo form

Please print and send the completed forms to the course registrar at below provided address. After we receive your completed application and permission forms, we will review your application and contact you.

If you are unable to print the forms, please contact the registrar with your name and address and application forms will be mailed to you.
